I know, that this doesn't really fit to all these classics, but my favorite depressive metal balad is pendulous skin from Mastodon.

Besides that my vote in general goes to Iced Earth - they have some awesome balads!

Cradle of Filth's thank god for suffering is quite sad.

also finnish rock band CMX has a shitloads of melancholic songs, but they sing in finnish

Agalloch from one end to the other... You can't listen to that band for more than an hour without having suicidal thoughts...

Also Love You to Death from Kamelot is fucking sad, I have shivers running down my spine every time I listen to that song. I have to mention most accoustic tracks by Into Eternity are so very melancholy, especially the 3 The Incurable Tragedy's on the eponyme album, they all talk about cancer, death and despair and the accoustic guitar has something so emotional.

If we talk about Moonspell, bets ''ballad'' can be Opium even its not so depresive, IMO Moonspell has more depresive lyrics how music, and Fernando is in My NR.1 in list whit whoom I wanna drink red wine
