Here's just 3 off the top of my head...

Sepultura - R.I.P. (Rest In Pain)

The outro to this used to scare me to death when I was younger! The last riff is derirative of Slayer, but a good one! After that, a seemingly normal crashing ending ensues, which then suddenly takes a carreening left turn into a twisted circus type melody, which leads into a final crashing ending featuring a maniacal tortured screaming horde before the voices slow right down and it ends with abrupt crackling sound that gives off the feeling that life has ended... yep, listening to that one at night in the dark with headphones was definitely a bad idea!

Iced Earth - Gettysburg (1863)

This 30+ minute epic certainly ends with an outro worthy of ending such an amazing musical piece. The final passage of Tim Owens singing despairingly "I look across this bloodsoaked land, all this blood is on my hands. God forgive me, please forgive me... It's all my fault, the blood is on my hands." is just searing, which leads perfectly into an ominous, distressed guitar riff with a highly dramatic horn section that sounds as if it's overseeing the death on the battlefield. It finally climaxes with a sombre and tragic sounding string section that laments the dead... I never fail to be moved by this section of the song.

Rush - 2112

Simply an epic ending, to an immensely epic song. "Attention all planets of the Solar Federation: We have assumed control"... I always get that spine tingling feeling when I hear that over the outro. Just amazing.

I love that ending to 2112, and the whole song in general! Nice pick.

I would add The Fountain of Lamneth in for Rush as well (from Caress of Steel). The similarities between the intro and outro tie the quite lengthy song together and end it all very nicely. It's a fantastic song.

Also, one of my favourite outro's ever is in the song "How Could I" by Cynic, off of Focus. After the last vocals are uttered there is a couple minutes of just a really nice flowing kind of riff and one hell of a solo over top of it. The song fades away over those last couple of minutes and it's rather ethereal.

The outro to Baying of the Hounds by Opeth really gets me pumped!! The most eargasmic part of this awesome song without a doubt. I also love the ending to the 30 minute epic The Great Escape by Seventh Wonder and how it all wraps up at the end, the acoustic guitar is chilling.

Gojira are really good at employing repetitive outros that often end up being the best part of the song. Some of the first ones that come to mind are The Gift of Guilt, In the Wilderness, Global Warming and The Way of All Flesh. But there are way more than that.

Other outros that stick with me are, off the top of my head:
Dark Tranquillity - Iridium
Sonata Arctica - Destruction Preventer
Katatonia - Tonight's Music
Porcupine Tree - Mellotron Scratch
Stryper - The Writing's On the Wall
Turisas - Five Hundred and One
Star One - It's Alive, She's Alive, We're Alive
Extol - Grace for Succession

Whenever a question like this comes up I always get carried away and end up trawling half of my music library :x

Also, nominating Pull Me Under for worst outro ever. What were they thinking?

For me it would be the Procession of Dead Clowns from The Work Which Transforms God by Blut aus Nord. Not sure if the others have not mentioned this song yet.
Also, White Cluster from Still Life and outro of The maniacal Vale are really excellent
