!T.O.O.H.! - Kali
We can close this now since I just posted the best way to end everything. This is how you end a song, how you end an album, how you end your entire carrier with your friends. Fuck it... This is how I want the fucking world to end!!! Words can't describe how brilliant and epic this outro is.
Damn man. That really is a phenomenal outro and your discription made me feel strangely emotional. It really would be a thing of absolute and inexplicable beauty to give the world such a magnificient ending. Just beyond words the awesomeness of it. I'd also mention Art of dying by Gojira and Floods by Pantera, both great outros though obviously it would be utter blasphemy to mention them even in the same breath as kali.

Still get goosebumps when listening Procession of Dead Clowns after all this time.... Best shit ever created.
A lot of other outros are worth mentioning, but I'm lazy now and it would be a long list.

Seconded for Blut Aus Nord, not that the song as a whole isn't great. I love that riff.

I probably couldn't pinpoint the most memorable outros for myself, but these two come to mind (I love my melodies):

Death - Crystal Mountain
The riffs, the drumming, double bass, and the acoustic playing as the song fades.
Gives me chill every time.

This one. The slow fading of the electric guitars into an accoustic part which in turn fades away into an existing song that was actually sung during the war. Simply brilliant imo. I always get shivers when hearing the outro of this song.

I'm not sure if you would consider them memorable outros, but hear is my top 5
1. The spoken part at the end of the song For Victory, by Bolt Thrower.
2. The grand finale part to 2112 by Rush which was an epic ending to an epic song.
3. The ending to Children of the Grave by Black Sabbath with those haunting vocals.
4. The finale to Deep Purple, Child in Time was hair-raising.
5. Furrows of the Gods by Drudkh, the ending melodic riff that the entire song built up to.
